Method of Calculating the Maximum Sum.

1. For the purposes of this Schedule the number of persons who are members of a Society or branch of a Society during any half-year after 1st July, 1918, shall be ascertained by adding the number of members of the Society or branch at the end of the half-year to the number of those members at the commencement of the half-year and dividing the total by two:

Provided that in the case of any Society or branch which is unable to show to the satisfaction of the Commissioners that its membership register is written up in such manner as to enable the number of members at the commencement and at the end of any half-year to be ascertained as aforesaid, the number of persons who are members of the Society er branch during the half-year shall be ascertained by adding the number of persons in respect of whom contributions have been, or are due to be, credited to the Society or branch for that half-year to the number of members discharged invalided from service with the Forces for whom contributions in respect of civil employment have not been or are not due to be credited to the Society or branch for that half-year.

2. The maximum sum which may be carried to the Administration Account of a Society or branch for the half-year out of contributions payable under the Act shall be the sum of amounts calculated in the following manner, that is to say:

(a) For every member of the Society or branch who is a seaman, marine, soldier or airman in the service of the Crown, the amount of 4d., or during the continuance of the present war 101d.;

(b) For every other member of the Society or branch the amount of 1s. 8td.:

Provided that a member who during any part of the half-year is included in Class (a) and during any other part of the half-year is included in Class (b) shall for the purpose of this calculation be deemed to have been included throughout the half-year in Class (b).

3. For the purpose of ascertaining the amount to be carried to the Administration Account of a Society or branch in any year, the year shall be deemed to be coincident with two half-years.

4. In this Schedule “member" does not include (a) a member who is of the age of seventy or upwards; and (b) a member who being a married woman has ceased to be a person whose normal occupation is employment within the meaning of Section 22 of the Act of 1918 or has on marriage ceased to be a voluntary contributor and in whose case more than twelve months have elapsed since the date of unemployment or the date of marriage as the case may be.

THIRD SCHEDULE.

Part I.

RESOLUTION by Approved Society to keep Joint Management Account. Resolved

1. That the under

Approved Society registered do keep a Joint Management Account in accordance with the provisions of the Regulations relating to Joint Management Accounts of Approved Societies, beginning with the calendar

2. That the purposes for which the Joint Management Account be kept be as follows:

(1) As respects the State business of the society all services relating thereto except the following reserved services*; and

(ii) As respects the private business of the society all services relating thereto except the following reserved services† ;

and that the cost of such services (other than the reserved services) be paid out of the Joint Management Account;

3. That the society do pay and transfer to the Joint Management Account for each year for which such an account is kept :-

(i) Out of the funds standing to the credit of the Administration Account in respect of the State business the sum of (here specify the rate of payment for each member or class of member); and

(ii) Out of the Management Fund in respect of the private business the sum of (here specify the rate of payment for each member or class of member); and

. 4. That the Committee of Management be instructed to take the necessary steps to carry into effect the foregoing arrangement and be empowered to accept such modifications thereof as the Insurance Commis sioners may require.

1. That the under do pay and transfer for each year for which the arrangement herein set forth continues to have effect in accordance with the provisions of the Regulations made under Section 17 (a) of the National Health Insurance Act, 1918, out of the funds standing to the credit of the State Administration Account to the Account kept in respect of the private business the sum of (here specify the rate of payment for each member or class of member) in consideration of the payment out of the Account kept in respect of the private business of the cost of all services required for State business other than the following reserved services, which services (other than the reserved services) shall be duly performed by the Society;

2. That the arrangement for the said payment take effect as from the beginning of the calendar year and continue for each succeeding year thereafter until terminated in accordance with the provisions of the said Regulations; and

3. That the Committee of Management be instructed to take the necessary steps to carry into effect the foregoing arrangement and be empowered to accept such modifications thereof as the Insurance Commissioners may require.

Part 11.

QUALIFICATIONS OF AUDITORS.

1. Membership of a body of Accountants incorporated by Royal Charter or of the Society of Incorporated Accountants and Auditors.

2. Inclusion in the List of Public Auditors appointed by the Treasury under the Friendly Societies Act, 1896, and the Industrial and Provident Societies Act, 1893.

3. Inclusion in a list of persons approved for service as auditors under a scheme of examination for persons who have had experience of Insurance administration or accounting, if the Commissioners have signified their approval of such scheme and of the standard of competency required thereunder.

FIFTH SCHEDULE.

PART I.

Procedure for convening and holding Special Meeting.

1. The Committee of Management of the Society or branch, as the case may be, shall, in conformity with the requirements of the Commissioners, not less than ten days before the date of the meeting, send to every member of the Society or branch a notice, stating the nature of the business to be transacted, together with a voting paper, and in addition to every insured member a statement containing particulars relating to the proposed amalgamation, transfer of engagements, or dissolution.

2. The meeting shall not be held earlier than six in the afternoon, unless the rules of the Society or branch otherwise provide, or the Commissioners otherwise determine.

3. A member shall be entitled to record his vote either at the meeting or by sending his voting paper properly filled up and addressed to the secretary of the Society or branch, but no voting paper so sent shall be valid unless delivered at the address set out in the notice of meeting not later than two clear days before the meeting:

Provided that the Commissioners may, if they think fit, upon the application of the Society or of any insured member thereof (or, in the case of a meeting of the branch, upon the application of the Society or of any member of the branch) extend the period of the time which must elapse between the delivery of the voting papers and the meeting.

4. All votes cast at the meeting shall be in writing and the voting papers used shall be similar to those despatched with the notice convening the meeting.

5.--(1) As soon as may be after the meeting the Society shall despatch to the Commissioners, or, in the case of a branch, the branch shall despatch to the Committee of Management of the Society

(a) all the voting papers, whether valid or invalid, returned to the Society or branch;

(b) a copy of any resolution passed at the meeting;

(c) a statement of the number of votes cast for and against any resolution by insured members and other members respectively; and

(d) such other particulars, if any, as the Commissioners or the Committee of Management of the Society, as the case may be, may require.

(2) In the case of the amalgamation, transfer of engagements or dissolution of a branch the Committee of Management of the Society shall furnish the Commissioners with such particulars relating to the amalgamation, transfer of engagements or dissolution as the Commissioners may require.

6. The notices and voting papers referred to in this Part of this Schedule shall be in the forms set out in Part II. thereof or in such other forms containing substantially similar particulars as the Commissioners may

approve.
