(5) If in the case of a constituent Society with branches the contingencies fund of the Society is not sufficient to make good any deficiencies disclosed on the valuation of any branches, a branch which has a deficiency remaining after the contingencies fund of the Society has been exhausted shall be treated as if it were a constituent Society of the Association and paragraph (2) of this Article shall apply accordingly.

On behalf of the Approved Societies named below we hereby apply for permission for the said Societies to form an Association, to be known as the Association, for the purposes of Section 3 (5)

of the National Health Insurance Act, 1918.

Certified copies of Resolutions passed by the several Societies for permission to form such Association and to become members of the Associa tion, if formed, are enclosed.

We hereby certify that a resolution that the above-named Society should join with other Approved Societies named in the application in applying to the Insurance Commissioners for permission to form an Association, to be known as the Association, for the purpose of M 2

Section 3 (5) of the National Health Insurance Act, 1918, and should become a constituent member of such Association, if formed, was duly a Special passed at General Meeting of the Society held on the the Annual day of

Application for Recognition as an Association.

To the Insurance Commissioners.

Association, being an Association of the Approved Societies named below formed for the purposes for which Provision was made by Section 39 of the National Insurance Act, 1911, and having been in existence [or having been in the course of being formed] on the first day of January, 1918, We hereby apply for the recognition of the said Association as an Association for the purposes of Section 3 (5) of the National Health Insurance Act, 1918; and We hereby certify that a resolution that the said Association should apply to the Insurance Commissioners for recognition as an Association for the purposes of Section 3 (5) aforesaid was duly passed by a majority of the members of the Committee of Management of the Association present and voting at a meeting of which due notice was given.
